Welcome to Memorial Health Network Connected Care Program! We are delighted that you have chosen to join our Remote Patient Monitoring (RPM) program, and we look forward to partnering with you on your journey to better health.

Here’s what you can expect from our Remote Patient Monitoring Program

Easy-to-Use Technology

We will provide you with simple, user-friendly devices to monitor your vital signs such as blood pressure, heart rate, and other relevant health data. These devices will automatically transmit your information to our healthcare team for review.

Continuous Monitoring and Support

With our RPM program, you can have peace of mind knowing that your health is being monitored around the clock. Should any urgent issues arise, our team will be alerted and take immediate steps to provide the care you need.

Improved Communication with Your Care Team

You’ll have more frequent and personalized interactions with your healthcare providers through phone calls, video consultations, and messages. This ensures that your health is continuously monitored and optimized.

Set Up Your Devices

A member of our team will be calling you to assist with the setup of your monitoring devices and ensure everything is functioning properly from the following number: 888-544-4567 (Please program this number into your contacts to avoid missed calls)

Begin Monitoring

Once your devices are set up, your daily health data will be sent automatically to our system. You don’t need to worry about manually entering anything.

Stay Connected

The goal is to take daily readings by 12:00 PM daily. Our team takes an approach through immediate outreach to you upon any abnormal readings or concerns. Our team is made up of licensed nurses and has a direct line of communication to your physician. Our team is available Monday-Sunday 8:00 AM – 4:00 PM PT.

Connected Care Availability

Connected Care is available to all eligible patients in service by Memorial Health. Please note, Connected Care is not a replacement for Emergency Services. If you believe you are experiencing a medical emergency, please call 911 or seek immediate medical care from the nearest hospital emergency room.
